Application for Undergraduate Readmission

This online form is for use by students who have previously attended Valdosta State University as an undergraduate student and would like to return as an undergraduate student.  Complete this form, attach required documentation (Verification of Lawful Presence documentation), and click 'SUBMIT.'  Your Readmission Application will not be processed until we receive the Verification of Lawful Presence documentation.  A photograph of your driver's license or identification card is the most common document used to satisfy this requirement. 

Readmission forms are due to the Office of the Registrar prior to the first class day of any semester. 

If you are currently on academic suspension and have not sat out the required time, do not complete this form.  Please complete the Suspension Appeal Form available on the Forms page.

    • You are required to submit Verification of Lawful Presence documentation prior to being approved for readmission.  Your application will not be reviewed until this documentation is received.  Documentation can be a GA drivers license, birth certificate, or a passport.  Additional information regarding Verification of Lawful Presence is available here.

    • False swearing statement: I understand that any material false statement made knowingly and willingly by me on this application, or any documents attached hereto may, in accordance with O.C.G.A. 16-10-71, which provides that upon conviction, a person who knowingly commits the offence of false swearing shall be punished by a ne of not more than $1,000 or by imprisonment for not less than one nor more than five years, or both, subject me to prosecution in a court of law. Additionally, I further understand that any such false statement may subject me to immediate dismissal from the institution.
